We are looking for a Clinical Care Coordinator (Unit Manager) that is patient focused that will enrich the lives of our residents, families and employees. This position will provide personalized, quality care and services to older adults that need assistance and oversee and manage the staff. We are searching for dedicated Nurse who has passion to serve the elderly. This candidate must be a team player, oversee the care partners and leads and have a positive outlook, able to problem solve and are caring and compassionate.
The responsibilities will provide nursing leadership to manage and direct the day-to-day operations of the skilled unit to ensure safety and quality of care, patient and family satisfaction, and staff satisfaction. Works collaboratively with the entire interdisciplinary team to plan, coordinate, implement, evaluate, and revise patient-related services and to meet the organizational mission, vision, and values. May be required to work as a Charge Nurse when necessary to include passing medications, performing treatments, monitoring care and overseeing staff. May be required to provide direct patient care to patients when necessary.
Performs annual reviews for unit-assigned staff. Responsible for education, coaching, mentoring, and disciplinary processes as applicable. Coordinates with scheduler to ensure that appropriate staffing levels are maintained for the provision of safe, quality care. Participates in the orientation process of new unit employees to ensure that competency requirements are met. Participates in the on-call schedule.
