Join our dedicated Assisted Living team as a Licensed Practical Nurse (LPN) lead on night shift. In this vital role, you’ll lead the overnight care staff in Assisted Living and Memory Care, provide hands-on nursing support, and ensure timely medication administration. The shift will be 10pm to 6am with a rotating weekend. This nurse will primarily support the Assisted Living and Memory Care teams, with occasional coverage in Skilled Nursing as needed.
Key Responsibilities
1. Assists residents with personal care and activities of daily living as assigned, in accordance with the service plan and state and federal regulations. Responds to resident calls and alerts promptly and adheres to assignments and care schedules.
2. Receives and gives accurate, timely verbal and written reports on residents' condition upon shift change. Confers with nursing leadership and/or the nurse(s) on the next shift to provide continuity of care and relay information to other nursing shifts to assure proper follow-up of physician's orders and instructions.
3. Makes routine rounds of assigned residents and modifies standard nursing procedures for care, as needed.
4. Ensures that medications are administered within required time frames and as needed (PRN). Documents any adverse reactions and takes appropriate measures.
5. Observes and reports symptoms/conditions of residents to the treating physician, director of nursing, and designated family member(s) in a timely manner.
6. Documents pertinent information in residents' medical records according to departmental policy and applicable State Department of Health regulations.
7. Serves as a role model and resource for other staff members and performs all other duties and responsibilities assigned to the Lead position.
8. Will be cross-trained to work in both Assisted Living and Skilled Nursing departments.
Requirements
- Active nursing license (LPN or RN) in good standing with the State of Michigan
- Basic computer skills
- Job duties will require frequent standing, walking, and lifting
