Clinical Pharmacist Specialist – Oncology – Ambulatory/Outpatient (POEM Program) 40hrs/wk

General Summary:This is a unique opportunity to provide direct patient care through oncology disease and medication management services in an established oncology center. This clinical pharmacist specialist (CPS) will work directly with oncology providers and other team members to provide direct patient care and education and serve as a resource to oncology providers and team members, under the direction of the Pharmacy Clinical Coordinator and the Director of Pharmacy Services. This CPS will provide consultation to physicians, nursing staff, and patients regarding the safe and effective use of medications, as well as medication management in the oncology setting to promote positive outcomes. This CPS will also be part of the statewide POEM (Pharmacists Optimizing Oncology Care Excellence in Michigan) network that provides support, education, and collaboration opportunities for POEM pharmacists. Experience in oncology, ambulatory care or hospital pharmacy is preferred, but not required. The network of POEM pharmacists and coordinating center will provide training and support for the UMH-West POEM pharmacist to expand their clinical expertise in oncology pharmacy and patient care.Essential Functions and Responsibilities:Provide oncology disease and medication management services to UMH-West oncology patients.Dedicate four days per week at the UMH-West Cancer Center delivering direct patient care and one day per week for program planning and development.Participate in regular conference calls with the POEM coordinating center for mentoring, training and follow-up.Attend quarterly meetings to learn of program updates, share best practices, discuss program successes and challenges, review quality data and outcomes, etc.Collaborate with other POEM clinical oncology pharmacists.Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, protocols, directives, or pharmacy operation standards.Provides education and information on pharmaceutical care and drug therapy.Contributes to the quality and effective operation of the department.Actively promote/participate in safety and quality improvementCommunicate effectively with both internal and external personnel.
