Person Centered approach to the planning and delivery of medical and personal services to residents, supporting residents' rights to participate in decision-making while treating residents with dignity and respect; directs and develops staff; collaborates with physicians and interdisciplinary teams; ensures customer satisfaction and adherence to policies, procedures, standards, and regulations.

Essential Job Functions:

· Ensure that proper resident care standards and practices are followed

· Communicates with residents, families and designated representatives about resident care issues

· Provides training, supervision, monitoring of staff as they provide personal care, medication management, and other health related services, assuring that philosophy of person-centered care is practiced

· Receives, reviews and notes physician and pharmacy orders, x-ray and lab reports and therapy recommendations. Assesses and reports change of condition to physician as necessary and appropriate. Draws blood as needed

· Ensures that residents' care needs are met by collaborating with physicians, discharge planners, nurses, home health agency staff. Arranges ancillary services, such as pharmacy, podiatry, lab, and x-ray as Regularly reviews and updates AFC Care Assessment plans, Care Agreements, medical records, activity plans and Care Guides

· Assures appropriate documentation is completed as required, verbally and written utilizing computer when necessary

· Understands and demonstrates effective and innovative approaches to challenging behaviors (expressions of unmet needs) by meeting the resident's unique needs and preferences. Utilizes resources ('care plan', team huddle, small group, family, reading material etc.) to address needs and then medications if needed. Documents effectiveness

· Supports Human Resources department. Provides employee immunizations and TB testing, responds to employee injuries

· Works with scheduler to ensure that appropriate number of staff is on duty for each shift while minimizing overtime. Creates daily assignments for staff utilizing the Care Guide
