General Summary:  Facilitates process improvement and coordinates improvement plans for the stroke program.  Provides the leadership and knowledge to continuously evaluate the patient experience and to improve service delivery for the stroke program. Will utilize multidisciplinary collaboration among various hospital departments to provide the highest quality, evidence based, patient-centered care.  Develops and maintains tracking methods for concurrent review of stroke patients to ensure compliance with all quality metrics.  Strategically develops, manages, and executes program activities focused on agreed upon timelines with outcomes that are timely and efficient and ensures that all accreditation requirements are achieved for stroke certification. Requirements: Bachelor’s degree in business, health services administration, healthcare informatics or other related education or equivalent combined education and work experience in a recognized health care or information technology field which is directly related to the duties of the position or equivalent combination of education and experience.Demonstrated expertise in health information management: analyzing and interpreting data in a health care related field; providing education to clinicians; and lead role in process improvement activities.Demonstrated expertise experience using data management software and visualization tools, including Microsoft Excel, Access, Word, PowerPoint, Project, Visio, SPSS, SAS, Tableau, etc.)Experience in creating and disseminating discernable, reliable, meaningful, and actionable data; experience in establishing, implementing, and analyzing interventions to improve process and outcome quality measuresAbility to assess stroke order sets and post assessments for external regulatory compliance.Demonstrated expertise in disease specific documentation design and reporting within the Epic medical record platform.Demonstrates critical thinking and analytical skills to understand root cause issues and how best to display information that is useful in creating opportunities for clinical improvement.Excellent organizational skills in setting priorities and balancing multiple priorities and demonstrated follow through bringing tasks to closure with acceptable outcomes.Excellent interpersonal, written and verbal communication skills with an emphasis on customer service.Ability to assess competing priorities, manage workflow and meet operational deadlines in a complex organization with minimal on-site supervision.Reasonable knowledge and ability to apply Lean concepts.Ability to work with a diverse group of people in a diplomatic and effective manner with demonstrated problem solving and conflict resolution skills.Demonstrated commitment to enhancing work place culture, embracing diversity and a commitment to creating the ideal work environment for faculty and staff.   Essential Functions and Responsibilities: Program Facilitation:  Responsible for development, performance and completion of complex projects involving multidisciplinary teams.Data Management:  Develops and maintains tracking methods for concurrent review of stroke patients to ensure compliance with all quality metrics.Epic/Vendor Liaison:  Collaborates with internal Epic and IT teams to optimize documentation efforts for clinicians, building of reports used for monitoring quality measurements and process improvement activities.Data Review/Process Improvement:  Thoroughly reviews clinical metric performance, assessing for trends and underlying causes for deficiencies for escalation and review by the CSC physician directors.Reporting:  Thoroughly reviews clinical metric performance, assess for trends and underlying causes for deficiencies for escalation and review by the CSC physician directors.On-Site Review:  In collaboration with the CSC Directors, serves as the HFAP CSC expert, providing guidance and assistance in preparation for on-site surveys by HFAP, as well as follow-up for areas of performance requiring attention identified during the survey.Education:  Provide education references to clinicians (physicians, nurses, NPPs, therapists, etc.) related to clinical metric performance requirements within CSC.
